PART 1: MANDATORY ADULT SPORTS LIABILITY WAIVER

SECTION 1: LIABILITY WAIVER, RELEASE, & INDEMNIFICATION

A. MULTI-SPORT VENUE & COMPETITION RISKS: I voluntarily choose to participate in sports tournaments, leagues, and athletic events hosted by E&E Athletics. I assume all inherent physical risks associated with competitive, high-velocity athletic play. These include, but are not limited to: collisions with other players, goal frames, backstops, or equipment; slick, hard, wet, or uneven playing surfaces (hardwood courts, indoor/outdoor turf, concrete, and natural fields); high-speed ball, puck, paddle, or object strikes; acute running, jumping, twisting, tumbling, or diving injuries; heat-related illnesses or severe outdoor weather exposure; and boundary hazards including perimeter fencing, walls, bleachers, nets, and scoring tables.

B. WAIVER, RELEASE & GOVERNING LAW: I expressly, knowingly, and voluntarily release, waive, and hold harmless E&E Athletics, tournament organizers, officials, referees, sponsors, facility operators, and on-site emergency medical personnel or event doctors from any and all liability, claims, or causes of action arising out of physical injury, permanent disability, or property damage, even if caused by the ordinary negligence of the released parties. This Agreement is governed strictly by Illinois law, with exclusive jurisdiction in the State Circuit Court where the athletic event occurs.

C. EMERGENCY MEDICAL AUTHORIZATION & FINANCES: In the event of an athletic injury or a sudden medical emergency, I hereby authorize E&E Athletics staff, referees, and the designated on-site event physician/doctor to secure and administer necessary emergency medical treatment, including but not limited to first aid, CPR, AED deployment, or ambulance transport. I explicitly acknowledge that I am solely responsible for all medical bills, diagnostic costs, and hospital fees incurred. E&E Athletics does not provide primary medical insurance for participants.

D. COMMUNICABLE DISEASE RELEASE: I understand that viral illnesses are highly contagious. I voluntarily assume the risk that I may be exposed to communicable diseases by attending or competing. I knowingly and freely waive, release, and hold harmless E&E Athletics and venue owners from any liability connected to illness transmission.

SECTION 2: PARTICIPANT CODE OF CONDUCT

Zero-Tolerance Abuse & Rules Compliance: I agree to maintain positive sportsmanship. Verbal abuse, screaming at, threatening, or making aggressive physical contact with tournament monitors, officials, judges, scoreboard operators, or opposing players is strictly prohibited. I agree to abide by all standard athletic regulations, including rules against illegal equipment or banned substances. E&E Athletics operates a Two-Strike Policy:

1. Strike 1 (Official Warning): A formal warning is issued to the individual and team captain.

2. Strike 2 (Scoreboard Penalty & Ejection): The abusive player is immediately ejected from the facility. Simultaneously, a mandatory technical foul / point penalty / yardage deduction / card will be executed directly against the associated team on the scoreboard. Refusal to leave the venue will result in an immediate team forfeiture and intervention by event security.

SECTION 3: PHOTO, VIDEO, & MEDIA RELEASE

I grant E&E Athletics and its legal representatives the irrevocable, perpetual, and unrestricted right to take, record, edit, and publish photographs, video footage, and audio recordings of myself taken during matches and event activities. This media may be used for any lawful commercial purpose, including but not limited to social media highlights, website galleries, live streams, marketing brochures, and promotional advertisements. I explicitly waive any right to financial royalties, compensation, or ownership claims over the captured media.
