Every player participating on a team registered by a team captain must complete an individual waiver before taking the court or field.

Important Information:

• All participating players must be at least 18 years old.

• Each player must submit their own waiver.

• Team registration and payment must be completed by the team captain.

• Submitting a waiver does not register a team or confirm tournament eligibility.

• Waivers must be submitted before the tournament’s published deadline.
