E&E ATHLETICS YOUTH SPORTS LIABILITY WAIVER & RELEASE

SECTION 1: LIABILITY WAIVER, RELEASE, & INDEMNIFICATION

A. OUTDOOR VENUE RISKS & RELEASE: I grant permission for my child to participate at E&E Athletics outdoor complexes. I assume all risks associated with outdoor play, including severe weather (lightning, extreme heat, heavy rain), UV exposure, and natural field hazards (uneven turf, holes, mud, loose dirt, or synthetic turf burns). I expressly, knowingly, and voluntarily release, waive, and hold harmless E&E Athletics, tournament organizers, officials, sponsors, facility operators, and on-site emergency medical personnel or event doctors from any and all liability, claims, or causes of action arising out of physical injury, permanent disability, or property damage, even if caused by the ordinary negligence of the released parties. This Agreement is governed strictly by Illinois law, with exclusive jurisdiction in the State Circuit Court where the tournament occurs.

B. INDOOR VENUE RISKS: I assume all risks associated with indoor play, including slick or hard court surfaces; boundary hazards such as walls, bleachers, nets, basketball standards, and scoring tables; and indoor environmental factors like lighting glare, architectural obstructions, or tight court spacing.

C. YOUTH ACTIVITY-SPECIFIC RISKS & INDEMNIFICATION: I acknowledge that youth sports involve varying levels of physical movement and inherent risks of severe injury. These include collisions with other players, goal frames, or equipment; acute injuries from running, jumping, tumbling, or diving; overhead stunt collapses; and localized risk from crowding the crease/goal mouth area in micro-pitch configurations. I agree to indemnify and defend E&E Athletics and its affiliates against any financial losses, damages, or legal fees resulting from lawsuits brought by myself, my child, or a third party regarding injuries sustained at the event.

D. COMMUNICABLE DISEASE RELEASE: I understand that viral illnesses are highly contagious. I voluntarily assume the risk that my child, myself, or my family members may be exposed to communicable diseases by attending. I knowingly and freely waive, release, and hold harmless tournament organizers and venue owners from any liability connected to illness transmission.

SECTION 2: EMERGENCY MEDICAL AUTHORIZATION & FINANCIAL RESPONSIBILITY

Consent to Treatment: In the event of injury or a sudden medical emergency, I hereby authorize E&E Athletics staff, referees, security, and the designated on-site event physician/doctor to secure and administer necessary emergency medical treatment, including but not limited to first aid, CPR, AED deployment, or ambulance transport.

Financial Responsibility: I explicitly acknowledge that I am solely responsible for all medical bills, diagnostic costs, and hospital fees incurred. E&E Athletics does not provide primary medical insurance for participants.

Refund Policy Deadline: Registration fees are non-refundable. Written cancellation requests received more than 14 days prior to the event will receive a future credit. No credits or refunds will be issued inside the 14-day mark.

SECTION 3: CDC & ILLINOIS STATE CONCUSSION COMPLIANCE

A. Acknowledgment of Educational Materials: I certify that I have downloaded, read, and understand the official CDC HEADS UP Concussion Fact Sheet for Parents (https://www.cdc.gov/heads-up/media/pdfs/youthsports/parent_athlete_info_sheet-a.pdf?CDC_AAref_Va) provided for this registration. I am aware of the signs, physical symptoms, and neurological risks associated with traumatic brain injuries.

B. Removal from Play Policy: I understand that if my child is suspected of sustaining a concussion, exhibits any signs of a brain injury, or takes a severe blow to the head or body, they will be immediately removed from play by officials, coaches, or event medical staff.

C. Return-to-Play Clearance: I agree that my child will not be permitted to return to tournament play under any circumstances until they have been thoroughly evaluated by a licensed healthcare professional trained in concussion management and have provided written medical clearance directly to the E&E Athletics tournament desk.

SECTION 4: PARENTAL & SPECTATOR CODE OF CONDUCT

Zero-Tolerance Sideline Abuse: I agree to maintain positive sportsmanship. Screaming at officials, judges, scoreboard operators, table monitors, or opposing children is strictly prohibited. E&E Athletics operates a Two-Strike Enforcement Policy:

1. Strike 1 (Official Warning): A formal verbal warning is issued to the spectator and head coach.

2. Strike 2 (Scoreboard Penalty & Ejection): The abusive spectator is immediately ejected from the facility. Simultaneously, a mandatory technical foul / point penalty / yardage deduction will be executed directly against the spectator's associated child's team on the scoreboard. Refusal to leave the venue will result in an immediate team forfeiture and intervention by event security.

SECTION 5: PHOTO, VIDEO, & MEDIA RELEASE

Consent to Media Capture & Commercial Use: I hereby grant E&E Athletics and its legal representatives the irrevocable, perpetual, and unrestricted right to take, record, edit, and publish photographs, video footage, and audio recordings of my child taken during the tournament. This media may be used for any lawful commercial purpose, including but not limited to social media highlights, website galleries, live streams, marketing brochures, and promotional advertisements. I explicitly waive any right to financial royalties, compensation, or ownership claims over the captured media. I also waive the right to inspect or approve the final finished digital or print product before it is published. I release and hold harmless E&E Athletics from any claims of privacy violations or right of publicity infractions under Illinois law regarding these images.
